Pasul II b): Înființarea Centrului medicilor de familie

Se aplică în cazul când mai mulți medici de familie titular optează pentru forma de activitate independentă Centrul medicilor de familie.

 

Pentru înființarea Centrului medicilor de familie administratorul ales de medicii de familie titulari va prezenta Dosarul cu următoarele documente la adresa electronică practica.individuala@msmps.gov.md

1) cerere prin care solicită  :

 

a) aprobarea Regulamentului de organizare și funcționare a Centrului medicilor de familie;

 

b) eliberarea Avizului pentru înregistrarea de stat a Centrului medicilor de familie.

 

La cerere se va anexa:

 

a) Regulamentul de organizare și funcționare a Centrului medicilor de familie, conform Regulamentului aprobat de către Minister (în 3 exemplare) ;

 

b) Declarația de fondare a Centrului medicilor de familie (copie);

 

c) Certificatele de atribuire a practicilor medicilor de familie fondatori (copii).
